I have received my insurance from www.insurancenoodle.com, underwritten by
Zurich. My rep (a year ago, I don't know if she's still there) was Jessica
Landini. 

The cost was approximately 1k$/year (yes, that low).

Also, there are companies who specialize in insurance for ISPs, such as 
www.ispinsurance.com, but I've had no personal experience dealing with
